Production Shoot Registration is Easier Than it Seems!

Don’t be daunted! Although the many forms and requirements under the Joint Administrative Order (JAO) regarding health and safety protocols for audiovisual production shoots may seem intimidating, registering your production shoot is actually easier than it looks!

Out of the nine downloadable forms under the JAO, there are actually only two forms required to be submitted for Safe Filming registration. These are Forms 1 and 3, or the Production Registration form and the Health and Safety Commitment Declaration Form.

Form 1, the Production Registration form, will be used to assist the Department of Labor and Employment (DOLE) in monitoring and the Department of Health (DOH) in contract tracing. This form will require the following: production company name, production company contract, project name, approximate shoot dates, and the names of essential cast and crew reporting on site.

Form 3, meanwhile, is the Health and Safety Commitment Declaration Form. This is simply a one-page form wherein the Production commits to providing and maintaining a safe and healthy workplace for all the workers.

These are the only two forms you need to get started with your production. All other forms will be required only if an incident or case of COVID-19 occurs within and during the production or to assist the production with such injury or illness-related emergencies.
